| Possible Side Effects | Gastrointestinal Upset: Nausea, vomiting, diarrhea, and loss of appetite are the most common adverse reactions. Injection Site Reactions: Pain, swelling, or inflammation can occur, particularly when administered intramuscularly (IM). Allergic Reactions: Hypersensitivity can manifest as skin rashes, hives, fever, facial swelling, or in rare, severe cases, anaphylaxis. Neurological: Seizures or CNS excitement may occur, though this is rare and typically associated with very high doses or rapid intravenous (IV) administration. |
| Drug & Food Interactions | Bacteriostatic Antibiotics: Drugs like tetracyclines, chloramphenicol, and macrolides (e.g., erythromycin) can interfere with the bactericidal action of ampicillin/sulbactam. Aminoglycosides: Antibiotics like gentamicin or amikacin can be physically inactivated if mixed in the same syringe or IV line with Unasyn. They can be used concurrently but must be administered separately. Probenecid: Decreases the renal excretion of ampicillin, leading to prolonged and higher blood concentrations of the antibiotic. Food Interactions: Because Unasyn is administered via injection (IV or IM), direct interactions with food in the gastrointestinal tract are not a primary concern, unlike oral medications. |
| Precautions | Small Herbivores: Unasyn should be strictly avoided in rabbits, guinea pigs, chinchillas, hamsters, and other small hindgut fermenters. Penicillins alter their normal gut flora, leading to life-threatening enterotoxemia. Hypersensitivity: Contraindicated in patients with a known history of allergy to penicillins or cephalosporins (due to potential cross-reactivity). Kidney Impairment: The drug is primarily cleared by the kidneys. Animals with severe renal disease require careful monitoring and potential dosage adjustments or extended dosing intervals to prevent toxicity. Pregnancy and Lactation: While generally considered safe for pregnant or nursing animals, the drug does cross the placenta and is excreted in milk, which may cause mild diarrhea in nursing offspring. It should be used only when the benefits clearly outweigh the risks. |
