Description

Cosyntropin lyophilized powder for injection (brand name Cortrosyn) is a synthetic form of the hormone adrenocorticotropic hormone (ACTH) used to test for adrenal gland disorders. As a lyophilized powder, it must be reconstituted with a sterile solution before being injected.

Uses

This medication is used as a diagnostic agent, typically in a medical test called an ACTH stimulation test, to screen for conditions such as:

Adrenal insufficiency: A condition where the adrenal glands do not produce enough cortisol. This includes:
- Primary adrenal insufficiency (Addison’s disease): Caused by a problem with the adrenal glands themselves.
- Secondary adrenal insufficiency: Caused by a problem with the pituitary gland, which produces ACTH.
Hypopituitarism: A condition where the pituitary gland fails to produce enough hormones.

Uses	Stimulates cortisol release: Cosyntropin mimics natural ACTH, which signals the adrenal glands to produce the hormone cortisol. Diagnostic agent: It is used in a medical procedure called the ACTH (or cosyntropin) stimulation test, which helps diagnose issues with the adrenal and pituitary glands. Evaluates adrenal function: The test measures how well the adrenal glands respond to stimulation. A blood sample is taken for a baseline cortisol measurement, and another sample is taken 30 to 60 minutes after the cosyntropin injection. If cortisol levels do not rise sufficiently, it may indicate a problem with the adrenal glands. Diagnoses adrenal insufficiency: This test helps healthcare providers identify conditions such as Addison's disease, where the adrenal glands do not produce enough cortisol, and hypopituitarism, where the pituitary gland fails to produce enough ACTH.
Contraindications	Cosyntropin for injection is contraindicated in patients with a history of hypersensitivity to cosyntropin or to any excipients of cosyntropin for injection. Reactions have included anaphylaxis. (4, 5.1)
Warnings and Precautions	Hypersensitivity: reactions including anaphylaxis have been reported. Monitor patients for hypersensitivity reactions and treat as needed. (5.1) • Diagnostic Inaccuracies: Cortisol levels and subsequent diagnosis of adrenocortical insufficiency following cosyntropin for injection administration may be inaccurate if patients are on certain medications because of their effect on cortisol or cortisol binding globulin levels. Any condition that elevates or lowers cortisol binding globulin levels may increase or decrease plasma total cortisol levels, respectively.
Adverse Reactions/Side Effects	Most common adverse reactions are: anaphylactic reaction, bradycardia, tachycardia, hypertension, peripheral edema, and rash (6)
Storage and Handling	Store at 20º to 25ºC (68º to 77ºF); excursions permitted between 15° to 30°C (59° to 86°F) [see USP Controlled Room Temperature Cosyntropin for injection is intended as a single-dose injection and contains no antimicrobial preservative. Any unused portion should be discarded.
